Resource Coordination and Patient Navigation

  • Resource coordination is the linkage and referral of a person to needed services.
  • Patient navigation is the process of helping to guide a person through the healthcare system. This includes help going through the screening, diagnosis, treatment, and follow-up of medical conditions related to the TBI.
  • Social workers responsible for resource coordination and patient navigation work together as a team to help identify needs and goals of individuals with TBI, then work to link the individual to services to help them reach their goals.
